The THA Board and TennCare are working together to implement THA’s ADT Service for all member hospitals by the end of 2017. The implementation strategy will focus on saturating THA Districts in order to maximize the benefits of the service by allowing member hospitals to receive readmission notifications for patients in their service area. THA will evaluate other priorities identified by the state including hospitals where patients attributed to primary care physicians that are participating in the bureau’s Patient-Centered Medical Home (PCMH) initiative are routinely served, Tennessee Assessment hospitals, and more throughout the year.
The below chart represents the target timing of each THA District, but changes are likely to occur throughout the year as more hospitals are participating and larger health systems with facilities in multiple Districts are on-boarded.
